CarolinaCard is located in the basement of the Russell House on Green Street, across from the student post office.

GETTING A CAROLINACARD:

  • Your first student/faculty/staff CarolinaCard (your permanent ID card) is issued at no cost. (One additional ID card will be provided free of charge in instances where an original social security number has been issued subsequent to originally issued ID card.)
  • To verify your identity, you must provide one of the following at the time your card is made:
    • United States Drivers License

    • Military ID

    • Passport

Spouses of Students/Faculty/Staff:

Effective January 1, 2013 there will be a $15.00 charge for all spouse ID cards produced, including spouse cards. In order to obtain a CarolinaCard spouse card, both the current USC student, staff, faculty member must be present, as well as the following:

  • Current USC student, staff, faculty CarolinaCard

  • Valid driver's license, military ID, or passport for student and spouse

  • Copy of your marriage certificate recognized by the State of South Carolina

If you do not have a copy of your marriage certificate and you were married in the state of South Carolina, you can obtain one through the Office of Vital Statistics. They are located on the first floor of the DHEC building at 2600 Bull Street. The cost is $12.00 for the certificate. If you were married outside of the state of South Carolina, you can call the county court system where you were married to obtain a copy.

For international students wishing to obtain a spouse CarolinaCard, all of the following must be presented:

  • Current USC student/staff/faculty CarolinaCard

  • Valid driver's license, military ID, or passport for student and spouse
  • Copy of your marriage certificate recognized by the State of South Carolina
  • Both sides of the I-20 form completed and either of the following:
     
    • J-1 form, "Certificate of eligibility for exchange visitor status"

    • F-1 form, "Certificate of Eligibility for Nominating Student"

*Note: International students can go to the International Student Office in order to obtain either of these forms.

REPLACING YOUR CAROLINACARD:
  • Replacement cards are $35 for students, faculty and staff.
  • Replacement cards for spouses are $25 for damaged cards and $35 for lost cards
LOST OR STOLEN CARDS:
  • Lost or stolen cards should be immediately reported to the CarolinaCard office, or reported at https://my.sc.edu/carolinacard AVAILABLE 24 hours a day. You are liable for any unauthorized use of your lost or stolen CarolinaCard, and all accounts on it, until we are notified. If you find your card, you can simply reactivate it the same way you deactivated it.
  • There is a $35.00 charge for a lost card/stolen card. If your card is stolen and it is not of your own negligence, there will be no charge for a replacement card when a Police report is provided.
